Medical Information
No matter how diligent we are in the care of your pet there is always a possibility they may have an unexpected illness/problem develop during their stay at our resort. The stress of being away from home and family, stress of other barking dogs, or a change in diet are just a few of the things that can cause problems with our pets. As part of our standard of care, we would like to address this potential problem at check in.

If problems arise during boarding, such as blood in urine, vomiting/diarrhea, coughing, etc., we will want to take care of your pet immediately. This may require additional tests and/or medications to properly treat the condition.

IMPORTANT:

constitute a confirmed pet boarding reservation. Please contact our office for availability.
All animals entering the hospital must be current on vaccinations (must have proof from a licensed veterinarian) and currently on flea prevention (must have proof of purchase) or they will be treated with a Capstar tablet at the owner’s expense. A Capstar tablet is a fast-acting flea treatment for use in dogs and cats that starts killing fleas in only 30 minutes.
Fees are charged on a per night basis. Pets are released only during normal office hours. Full payment is due upon release. If you are scheduled to pick up after hours, additional fees will apply.

* I understand All for Pets Veterinary Clinic strives to provide the best boarding experience possible and will act reasonably and with regard for my pet's well-being, should a need arise. I understand that All for Pets does not provide or recommend toys/swallow hazards in living areas causing a potential for destruction and ingestion of said items. I will not hold All for Pets Veterinary Clinic accountable for any and all inconveniences, expenses, or any other problems my pet might experience during or after boarding due to destruction or ingestion of personal belongings.
* I am aware that boarding/day care/grooming may cause stress, which can lower the immunity of my pet. I realize my pet's vaccines and the vaccines of other pets in the kennel, along with cleaning and disinfecting may not prevent all common ailments associated with boarding/daycare/grooming, and I will not hold All for Pets Veterinary Clinic accountable for any inconveniences, expenses, or other problems that my pet might experience during or after boarding/daycare/grooming.
* I authorize the veterinarian to do whatever necessary should an emergency arise, to include tranquilization as required. I agree to pick up my pet within 5 days of the discharge date, and my pet may be considered abandoned if I do not contact All for Pets within the designated time. In my failure to recover my pet, you are automatically authorized to dispose of my pet(s) as deemed professionally necessary. Pets are released only during normal office hours. Full payment is due upon release. If you are scheduled to pick up after hours, additional fees will apply.
